Health Requirements

• Must be 65 years of age or older
• Must be without a primary or secondary diagnosis of chronic mental illness or diagnosis of Alzheimer’s disease
• Negative Tuberculosis Test Result upon admission into facility with the last 90 days
• Must have at least one need for assistance with daily living.

• Public Assistance Verification
• Long Term Insurance Benefits
• IRA/ Keogh Accounts
• Disability Benefits
• Whole / Universal Life Insurance Policy
• Annuities/ Money Market Accounts
• Copy of picture I.D. or Driver License
• Copy of Medicare Card. Medicaid Card
• Birth Certificate
• TB Test Prior to Move In
• Bank Statements- copy of last 3 years ( Checking & Savings)

Income Requirements

Pioneer Gardens is designed to offer residents who qualify the opportunity to access Medicaid benefits or pay market rent for our apartments.  Sales & Marketing will provide more detailed information upon consultation and tour of the facility.
